Not All Movement May Be Equal for the Brain

Good Morning.

Ten thousand steps is ten thousand steps.

At least to a fitness tracker.

But a massive new analysis suggests that when it comes to long-term brain health, where and why we move may matter alongside how much.

Researchers combined 74 studies involving more than 4.2 million adults across 30 countries. They separated physical activity into four parts of everyday life: exercise during leisure time, physical activity at work, household activity, and active commuting. Participants were followed for a median of 10 years.

The results produced an unexpected divide.

People with high levels of leisure-time physical activity had a 24% lower risk of developing dementia than those with the lowest levels.

But people who were highly active at work showed the opposite association. Their dementia risk was 20% higher. Active commuting was also associated with a modestly higher risk.

That doesn't mean physical work or walking to work causes dementia. The studies were observational, and the researchers can't yet explain the difference.

But consider what two apparently similar hours of movement can contain.

A walk taken by choice might include changing terrain, conversation, time outdoors, and control over pace. Movement during a physically demanding job can involve repetitive tasks, prolonged standing, deadlines, noise, pollution, and less control over when to stop.

The muscles may be active in both situations. The larger experience is very different.

Researchers have sometimes called this the physical activity paradox: exercise during leisure time is consistently associated with health benefits that physical activity on the job doesn't always appear to provide.

This new analysis suggests that the distinction may extend to the brain.

We've become very good at measuring movement in steps, minutes, and calories.

But perhaps those numbers leave out something important.

The body knows not only that it moved.

It also experienced everything that came with the movement.
